This training is for OhioRISE Care Management Entity Care Coordinators. This training will be a mix of didactic learning and experiential learning to equip participants with effective care coordination facilitation skills.

This training is only for those who are certified in a previous version of the CANS and need booster training on the new Ohio Children’s Initiative. Prior- Approval required after verification of certification status.

Office Hours provides individual technical assistance regarding application of the tool and/or application of the tool within system initiatives. Office Hours allows providers an opportunity to build additional competency in practice applications, special populations and other special implementation considerations. Bring your questions!
