Wraparound SoC | Moderate Care Coordination (MCC)

Building off of the 4 Wraparound Process Pillars, this training will take a deep dive into the implementation of Moderate Care Coordination in OhioRISE. Participants will learn about the 5 key functions of a Moderate Care Coordinator and how to implement those functions throughout the arc of care with families. Tools and strategies such as: completing a Capacity, Assets, and Resiliency Examination; building a Child and Family Network; Laura Bernstein's 'Model for Taking Action'; Reg Revan's 'Action Learning Cycle'; and effective transition planning; will all be discussed and demonstrated.
