Use this form to link what the person says to an underlying needs statement. Identify what each family member said to you taking care to list exactly what they said. Then work through each of the columns until you get to the end of the row. The last two columns in each row will reflect needs statements that can be brought to the first Child and Family Team meeting. Lead the team in selecting those needs statements that will help the team accomplish the mission.
