Use this form to create a Communication Contact Tree during Team Meeting. Each Team Member will develop their own form. Identify who to call first and then put the communication plan in place. Each person calls the person below them. If no one is there leave a message and call the next person the list. Ask team members to identify their name and number in each box moving down the table. When the boxes are filled out assure the parent and youth sign and approve each member’s Communication Tree. You can tab through each boxes in filling this out or print and have it filled out by hand. When a new Team Member joins have them develop their Communication Tree.
